Genentech BioOncology Co-pay Card Program

At Genentech, we are committed to helping patients access the treatment they need.
That's why we offer the Genentech BioOncology Co-pay Card. If you are eligible, this card can help lower your out- of pocket costs like co-payments, co-insurance or deductibles for your Genentech cancer therapy. Out-of-pocket costs are the part of your treatment that your healtcare plan requires you to pay.

What if I am not eligible?
If you answered “no” to any of the questions above but need help paying for your treatment, we can still help you by referring you to an independent, non-profit organization (INO) for financial assistance. If you do not have insurance, you may be able to get free treatment through the Genentech Access to Care Foundation. Call a Genentech BioOncology™ Access Solutions® Specialist at 1-888-249-4918 from 6 am – 5 pm PT, or visit www.BioOncologyAccessSolutions.com to find out more.

- We cannot guarantee you will get help from an INO. These INOs each have their own rules, including financial rules. Genentech does not influence or control these INOs. But Genentech BioOncology Access Solutions can help you. We can refer you to an appropriate INO. We can also help with the application process.
How can this card help me?
Your out-of-pocket costs can add up fast. That’s why we offer assistance who have trouble paying for their treatment.

| Card limit | Up to $4,000 per year* |
| That means that if your out-of-pocket costs are $100, the card covers $80 and you pay the other $20. | |

- If your yearly household income is >$100,000 your card limit is $1,500 per year.
You are only required to provide a verbal statement to verify your income at the time of enrollment, but you may have to provide financial documentation at a later date.
How do I enroll?
Your out-of-pocket costs can add up fast. That's why we offer assistance to patients who have trouble paying for their treatment.

| 1. Visit www.ActivateTheCard.com/AVASTIN or call 1-877-344-7774 from 9 am-8 pm ET, Monday to Friday |
| 2. Answer a few simple questions, including your name, address, phone number and date of birth. |
| That's it. After these 2 steps, if you are eligible, your card is activated. |
Terms and conditions
Use of the card does not obligate you to use or continue using any specific product or provider.
You may receive additional program information about the Genentech BioOncology Copay Card once you are enrolled. Genentech BioOncology Access Solutions can also activate this card for you.
No person or entity may seek reimbursement from any third-party payer for any amount provided using the card program. Use of this card must be consistent with all relavant health insurance requirements and payer agreements. Participating patients and pharmacies are obligated to inform third-party payers about this card as provided for under the applicable insurance or as otherwise required by contract or law. Genentch USA, Inc. reserves the right to deny payment under the card to anyone deemed ineligible in accordance with the stated program criteria.
